Our plan for growth in 2025
In 2025, Brisbane South PHN will continue to strengthen health systems, respond to community needs and drive innovation in primary healthcare. Our priorities are to expand access, build partnerships and deliver services that meet the needs of our diverse communities. We remain committed to advancing health equity and responsive care, aligned with national goals for strengthening Medicare and improving local healthcare.
Expanding access to integrated, person-centred care
We will work to make healthcare more accessible and better coordinated for everyone in our region by extending support and improving preventative and data-informed care.
Key initiatives include:
- opening a third Medicare Urgent Care Clinic in south-west Brisbane to further improve access to non-emergency care
- increasing MyMedicare and Primary Sense adoption to support more general practices in delivering coordinated care and improving patient outcomes.
Supporting First Nations and multicultural health
Brisbane South PHN is dedicated to ensuring culturally safe and respectful care for all. As the lead PHN for multicultural health, we will continue developing frameworks to support fair access to healthcare across communities. We will also build on our First Nations health programs and deliver on our Stretch Reconciliation Action Plan actions, aimed at improving health outcomes for Aboriginal and Torres Strait Islander peoples.
We want to:
- enhance the Multicultural Commissioning Framework to support culturally responsive care across PHNs
- grow our First Nations Workforce Development initiatives, including programs to help young First Nations peoples build careers in health.
Expanding mental health initiatives and family support services
We will strengthen mental health and family support services in 2025 through our Local Link programs and other co-commissioned services.
We’ll focus on:
- building trauma-informed recovery services by commissioning new programs to support people affected by domestic and family violence
- expanding community mental health partnerships to ensure timely, accessible mental health support.
Building capacity through innovation and technology
In 2025, we will continue to improve our data and digital health capabilities, helping our healthcare partners access insights and resources to deliver high-quality care.
Upcoming innovations include:
- new practice reports for general practices using Primary Sense, including chronic disease, cancer screening and preventive health data
- releasing a Digital Health Quick Help Guide to support practices in adopting digital solutions for better care delivery.
Strengthening partnerships and regional healthcare collaboration
Collaboration remains central to our strategy. In 2025, we will continue working with Metro South Health, local councils and community organisations to address health needs in our region.
Our partnership goals include:
- implementing recommendations from the Joint Regional Needs Assessment in partnership with Metro South Health to address gaps in access to healthcare
- supporting early childhood and family programs through collaborations such as the TOTs Initiative, providing critical support to children and families.
Continuing commitment to organisational excellence
Our Capability Framework and quality management systems reflect our commitment to high organisational standards. In 2025, we will keep strengthening our systems, governance and workforce to achieve our mission and deliver sustainable, effective services.
